Atheltic Physicals

Parents/Guardians,

If your child plans to participate in sports during the 2024-2025 school year, they are required to have a sports physical prior to the start of any season. Albany Area Primary Health Care, Inc. will be conducting athletic physicals at Pataula Charter Academy on Thursday, April 11, 2024.

These exams will be performed at a cost of $25 per student. Parents will be responsible for the cost of the ($25) sports physical. (Make checks payable to Albany Area Primary Health) These exams may also be billed through insurance. If your child is due for a well child exam, they will perform the well child exam and fill out the sports physical form for your child. 

We STRONGLY encourage you to take advantage of this convenient opportunity. If you are interested in having your child scheduled for their sports physical, please fill out the following forms completely and return them on Tuesday, April 9.

• Provider Clearance Form (Parent/Guardian Signature Required)
• History Portion of the GHSA Pre-Participation Physical Evaluation Form (Page 1 & 2)
